Instructions for Completing the Security Sponsorship Form

  Please use this form to sponsor a Shabbat or event for Congregation Beth Shalom's security initiative. Sponsorship helps fund our professional security presence, ensuring our synagogue remains safe and welcoming.

To complete your sponsorship:

1. Enter Your Full Name Fill in your first and last name in the designated fields.
2. Provide Your Email Address This allows us to send confirmation and communicate regarding your selected sponsorship.
3. Select Your Sponsorship Amount Use the dropdown menu to choose your donation amount. Options start at $36 and increase to $180, with a suggested price of $180 for full sponsorship.
4. Total Contribution The total amount will calculate automatically based on your selection.
5. Submit Your Form Once all fields are complete, click “Submit” to finalize your sponsorship.
